Review

"Power, Politics, and Universal Health Care is a first-rate analysis, skillfully tracing the political, social, and economic forces that, for nearly a century, thwarted efforts to enact universal health insurance legislation. At times, it reads more like a novel than a policy book as it tells the fascinating story of how the forces that frustrated past efforts to provide universal insurance coverage were overcome to pass the Affordable Care Act."
-Robert D. Reischauer, president of the Urban Institute and former director of the Congressional Budget Office, 1989–1995

"A riveting journey through the history of US health care reform. This book is a must-read for anyone who wants to understand America’s elusive search for universal coverage and affordable care. Altman and Shactman do the impossible—make sense of our complex health system in an accessible and compelling way."
-Jonathan Oberlander, PhD, professor at the University of North Carolina at Chapel Hill and author of The Political Life of Medicare

"Stuart Altman and David Shactman’s new book does a superb job of capturing the essence of the meandering odyssey of health care policy.... They describe the pivotal events and characters of the development of universal health care with the intimacy of good storytelling. The authors make you feel like you are there."
-Charles N. Kahn III, president and CEO of the Federation of American Hospitals and former staff director of the House Ways and Means Health Subcommittee, 1995–1998

"There is not an American who has been more active and relevant in the health reform debate for the last forty years than Stuart Altman. With his talented partner, David Shactman, he has produced a well-written, insightful personal recollection of the evolution of health reform. It is an invaluable contribution to understanding how all major reforms are built on the triumphs and failures of past attempts and cannot be achieved without the application of lessons learned, leadership, good timing, and luck."
-Chris Jennings, former senior health reform adviser to President Bill Clinton, 1994–2001

"Rendered more in the riveting prose of a spy novel than in the turgid text that usually emits from academia, the authors present an insider’s narrative of the major defeats and small victories in the century-old quest to provide all Americans, rich and poor, financial and physical access to timely health care without bankrupting their families."
-Uwe E. Reinhardt, PHD, James Madison Professor of Political Economy at the Woodrow Wilson School at Princeton University

About the Author

Stuart Altman, the Sol C. Chaikin Professor of National Health Policy at the Heller School for Social Policy and Management, Brandeis University, is an internationally recognized expert in health policy. He served as a deputy assistant secretary of Health, Education, and Welfare in the Nixon administration and was one of the architects of Nixon’s plan for universal health care. He was chair of the Prospective Payment Assessment Commission (ProPAC) for twelve years under presidents Ronald Reagan, George H. W. Bush, and Bill Clinton. ProPAC was responsible for overseeing the way Medicare paid hospitals and other health institutions. He also served on President Clinton’s transition team and was appointed to the president’s Bipartisan Commission on the Future of Medicare. Recently, he was a member of the health policy team for Barack Obama during his presidential campaign.

David Shactman is a freelance writer who was a senior fellow at the Schneider Institute for Health Policy at the Heller School for Social Policy and Management, Brandeis University, and worked with Stuart Altman for eleven years. During that time, he was coeditor of two books: Policies for an Aging Society (with Stuart Altman) and Regulating Managed Care (with Stuart Altman and Uwe E. Reinhardt). He has also written numerous articles published in peer-reviewed journals and trade magazines, including Health Affairs and the New England Journal of Medicine.

Altman and Shactman have given us an engaging, comprehensive account of the history of federally managed universally available health care in the United States, but Altman is the central figure and narrator.
The idea of universal health care has circulated among Progressives for more than a century, starting with Teddy Roosevelt. The path included some turns that seem surprising to us now, such as Samuel Gompers, the early 20th century labor leader, opposing it on the grounds that it would weaken the need for unions. Franklin Roosevelt studied the idea, but chose not to pursue it because he felt that it would detract support from his proposal for Social Security. These are examples of the political insights that Altman adds to the historical account.

The modern story begins with Richard Nixon who, according to Altman, came closer to establishing a national health program than any president before Obama. Nixon worked with Senator Ted Kennedy and Congressman Wilbur Mills to enact a program crafted in large part by Altman, who at the time was a deputy assistant secretary of Health, Education, and Welfare. It might have passed except for Mills being caught in in a Washington reflecting pool with a lady of uncertain reputation, Nixon becoming embroiled with Watergate, and Kennedy's continuing problems with Chappaquiddick. Nixon did, though, manage to enact the Health Maintenance Act of 1973, a "historic" piece of legislation that greatly expanded the availability of HMOs by requiring all employers to offer them if any health plan was provided.

The story continues with the Hill Burton Act, which requires hospitals to treat indigent patients up to a certain fraction of their business.

After hearing several positive things about Blueprints Psychiatry, I decided to go to the store and buy it. Didn't want to work too hard, didn't want to look like a total moron. Then I picked it up, and I think I've seen bulkier books written by another doctor (Seuss, that is). But brevity isn't necessarily bad, so I checked it out at the library instead and read it in 3 days of semi-hard studying. Yeah... I think should've gone with the other book, Green Eggs and Ham.

The problem with Blueprints is that it largely rehashes DSM-IV guidelines. That's OK, and it's a big part of psych, but you can get that with the Current Clinical guidelines pocketbook. What I was really expecting/wanting was something that (1) had bits of annoying trivia pertinent to the exam (2) more information on psychoactive drugs (3) described in more detail patient presentations and (4) discussed how other medical diseases could present like psych disorders. Blueprints skims these issues.

I would buy First Aid for the Psych Clerkship instead. It's a quick read as well but provides more info. I had a good score on the psych shelf, and I think switching primary sources had a great deal to do with it. If you like the Blueprints series, just check it out or borrow it from a friend. Blueprints Q&A, though, is an excellent question resource.

This book is concise and easy to read to cover psychiatry for USMLE step 2. The description of each topics are straight to the point, and focusing on the details that appear in the exam. The biggest weak point of this book is that it lacks clinical viganettes, so it can be hard for you to get the image of each disorders. So if you can get one of those clinical viganette books, that will be perfect. Additionally, they are missing some important topics, such as sleeping disorders, but still, this is the best available at this point.

Though Foucault is now widely taught in universities, his writings are notoriously difficult. Reassessing Foucault critically examines the implications of his work for students and researchers in a wide range of areas in the social and human sciences.
Focusing on the social history of medicine, successive chapters deal with his historiographical, methodological and philosophical writings, his ideas about prisons, hospitals, madness and disease, and his thinking about the body.
The book also suggests ways in which Foucault's influence will continue to dominate cultural history and the social sciences.

I am pretty certain that if anyone reads this book they will be worse off because of it. i tried to compare some of the info from this book with other material, but this book is just such a waste of time. even if i had all the time to study this book, i wouldnt. this book wont help you be a better doctor and it wont even help you on the exam. rather, it will take away from both.
By Review
I recommend all doctors to read this book before writing Step 3 CCS. I tried various resources (Uworld, USMLE consult) but nothing really quenched my fear. But after reading this book I am feel very confident about my preparation. Dr. Fisher has done great job explaining all clinical scenarios in time and space with a thorough explanation. This book not only meant to get you through the USMLE Step-3 but it is full of "must know clinical medicine."

I have a background in the performing arts and was terrified of changing my career path toward Occupational Therapy after not being in school for ten years, mainly because of the dense science courses. I bought this book and worked through it over the summer before starting Anatomy and Physiology I and II prerequisites the following year. I was so pleased with this-- I needed a watered down, interesting version before starting back up and diving into the dense text my class required. I am a visual, sensory learner and this book helped me greatly. I got A's in the Prerequisites--master's degree here I come!

As an undergraduate text for medicinal chemistry, I think this book is the best in the market. It's appropriate for students who have only had two semesters of organic chemistry. The layout is logical. Aside from chapter 7 (chelating agents), the first nine chapters are at least decent and mostly very good. Chapter 10 (nucleic acids) and 11 (nitric oxide) probably belong more in a biochem text than a med chem book. Chapter 12 (organic synthesis) is well out of the scope of this book and falls flat.
The book has a huge number of errors. Most of which are in the chemical figures and will not bother anyone unless they like looking at structures. Some of the errors involve the mathematic equations and could cause trouble for a student who isn't paying attention.
The only major complaint I have about the book is that chapter 5 (pharmacokinetics) is almost completely devoid of units on the variables in the equations and graphs. Proper use of units would greatly help a student in learning material in this chapter.
While not perfect, this book is very good for a junior/senior upper-level chemistry course.
By A Customer
This textbook by Thomas is a very big disappointment to me. I am still trying to see how so little useful information can be spread over a vast 620 pages. The subtitle "An introduction" is one thing, but for the author to spend so much time discussing background without giving precise information about how the molecules act as drugs is unfortunate. In so many areas, it is just a waste of the students' time. It has poor illustrations too. The book is inexpensive, but I would rather pay more for a book that I could actually learn some interesting information about. If you wish to learn medicinal chemistry, you need a better book than this. This book give almost no insight in the strategies that a medicinal chemist actually uses to design drugs. Fortunately, there are some very good books out there today. My three favorite MedChem books are those written by: Patrick, Silverman, and Sneader....in that order. Nogrady is also a good book but it suffers from the opposite extreme that this book by Thomas does (it has too many specifics and not enough background).

This is a required text for one of my BSW classes and I wish I hadn't wasted my money purchasing it. The book is not constructed in a clear way, topics run together with no clear transitions, and is poorly written. The author will introduce a theory or define a term by quoting other authors' definitions. Not just one per term, but many for each new term explained. By the time you finish reading that paragraph you have multiple views on a definition, and none of them are clear. I have read over 40 college textbooks and this is by far the worst I have ever had the misfortune to read. The writing style of the author is so atrocious it appears to be a research paper gathered by a fifth-grader!
By T. Hufnagel
This book is as clear as mud. I read this book to better understand alternative paradigms that are all too often missing from others on this subject. Unfortunately, the author focused more on including everyone's perspective than making any idea clear.

John P. McGovern held seventeen professorships, received twenty-nine honorary doctorates, and established the nation’s largest privately owned allergy and immunology clinic. He authored 252 professional publications including twenty-six books in the medical sciences and humanities, and served as president or chief elected officer of fifteen professional societies in medicine. In addition, the McGovern Foundation has given millions of dollars to various local and national health charities, and many Houston landmarks bear the McGovern name, including the McGovern Lake and McGovern Children’s Zoo (at Houston’s Hermann Park), the McGovern Health and Science Museum, and the McGovern Campus of the Texas Medical Center.

Bryant Boutwell, a long-time friend and colleague, has captured the influential life of this visionary Texas physician in John P. McGovern, MD: A Lifetime of Stories. In captivating narrative, interlaced with revealing personal and family stories, Boutwell chronicles McGovern’s holistic approach to medicine, which transcended the traditional boundaries of institutional identities and medical specialties. McGovern worked tirelessly to bring together big institutions, the health professions, bold interdisciplinary ideas, and a team approach to healthcare that, though prescient at the time, is recognized today as imperative. This commitment led to his founding role in the American Osler Society, which promotes humanistic and ethical dimensions of the practice of medicine, and his establishment of humanities programs at the UT Health Science Center at Houston and the UT Medical Branch at Galveston.

Given the glowing reviews, I was genuinely surprised by the actual book.

It is very badly formatted and seems to have had little, if any, professional editing and organizing. Some of the text is printed in a dark grey box, which makes it nearly impossible to read. There are long paragraphs which really should've been broken up. There is some use of lists and bullets, but not nearly enough. The font is strangely round, and the page looks very cramped.

I can't help but think that this book seems to be nearly self-published. If you, like I, have recently been reading any of the NOLO books on starting your own business, this feels like a serious step down in presentation. (There is a reason for an author to work with editors and publishers: they make the book more readable.)

As for the content itself, I find it middling. There are pages and pages on having a trademark, but hardly any details on how to choose malpractice. Again, seems to be lacking organization and editorial vision.

I almost never return a book, but this one I am sending back.
By lesliac
Great book. Part I gives a great overview of how to start your own medical practice based on Hacker's experience as an entrepreneur. Should be noted, though, that Hacker does not discuss anything about creating a cash practice.

Part II of the book discusses how to start and run your own business outside of medical practice. I purchased the book because of Part I, but Part II was a great read as well.

To summarize, I think this book should be required reading in medical school to educate American physicians on how to be successful in running a practice. I'm content working where I'm at now, but should I ever decide to change locations, I'll refer to this book to guide my future practice.

Thank you Dr. Hacker for taking the time to write this book!

This book is by one of the first authors to recognise that communication skills are important in science, and that they are a craft that has rules and principles that any student can learn relatively easily. It deserves to be treated as a classic, and should be compulsory reading for any student of the life sciences.
I can recall reading it with much interest during my PhD in the 1980's, when it was about the only work available in this field, and various biochemical societies in the UK made it available to students. Much of the advice on straightforward communication style is still extremely valid today. In the later 80's and 90's, the field of writing skills took off, and now most University bookshops have several shelves of books on writing skills and how to write a thesis. Booth's book is short, direct, and the advice is very practical. Read it!

Desperate Marriages:
Moving Toward Hope and Healing in Your Relationship
by
Gary Chapman
Review by
Anthony J. Centore Ph.D.

* * *
After Gary Chapman wrote the International Best Seller The Five Love Languages, myriad of spinoffs were published: The Five Love Languages of Children, The Five Love Languages of Teenagers, The Five Love Languages for Singles, The Heart of the Five Love Languages, The Five Languages of Apology, and The Love Languages of God; not to mention what appears to be several updated revisions of the aforementioned.

Rest assured, this book barely mentions the five love languages--keeping it to a minimum of just two pages in the first chapter, two pages in the back. Done. Finished. Life moves on. Finally.

In Desperate Marriages, Chapman promotes a philosophy he refers to as "Reality Living," of which there are six rules: (1) I am responsible for my attitude, (2) My attitude affects my actions, (3) I cannot change others, but I can influence others, (4) My emotions do NOT control my actions, (5) Admitting my imperfections does not mean I am a failure, (6) Love is the most powerful weapon for good in the world.

Basic Impressions:

This book is well put together. It is quality material. The reader with real marital problems and needing real information will get real information. For instance, the book speaks about divorce in chapter one--a smart move, for this is certainly on the mind of someone who is in a desperate marriage. Chapman states, "while divorce removes some pressures, it creates a host of others." Moreover, the book contains some good psychology (which any counselor reader will recognize as being of the CBT persuasion), and Chapman aptly references William Glasser (i.e.
If you've read Chapman before, you're probably aware of his concept of "love languages." In a nutshell, the idea is that everyone has one or more ways that they most need to "experience" being loved. These languages are: Words of Affirmation, Quality Time, Giving Gifts, Acts of Service, and Physical Touch. Chapman believes that if we challenge ourselves to love our spouses using THEIR love language -- and they do the same for us -- marital harmony will result.

In Desperate Marriages, Chapman walks couples through applying these concepts to the most difficult of situations: marriages in crisis. In these situations, it's not quite as simple as just using the right love language. Usually, one or more myths stand in the way of the love language exchange.

He identifies four "myths" which he believes will make it impossible to save a desperate marriage:

1. My environment determines my state of mind.
2. People can't change.
3. In a desperate marriage, I have only two options -- either resigning myself to a life of misery, or getting out.
4. My situation is hopeless.

But then he counters by offering six "realities," or principles, which bust those myths and can lead to marriage-transforming change. He calls the application of these principles "Reality Living." They are as follows:

1. I am responsible for my own attitude.
2. My attitude affects my actions.
3. I cannot CHANGE others, but I can INFLUENCE others.
4. My emotions do not control my actions.
5. Admitting my imperfections does not mean that I am a failure.
6. Love is the most powerful weapon for good in the world.

Chapman uses conceptual these tools of myths, realities and love languages to show how to repair a variety of desperate marriages.

By Mouzhan Mangum
I do not usually make the time to read 371-page books, but found myself showing up early to doctors' appointments to arrange protected time to finish this book. It is well-organized and contains a treasure trove of practical information for healthcare organizations at every level of readiness. In fact, a table on page 153 shows key concepts and actions to consider for organizations that are considering forming an Accountable Care Organization (ACO):
* Interested organizations should engage physicians in designing and managing integrated clinical functions, such as service lines, and in collaborations to improve operational efficiency, for example, reducing length of stay and readmissions;
* Engaged organizations should reward introduction of care pathways in common clinical areas and build physician management in the ambulatory environment
* Committed organizations should focus on strengthening inpatient and ambulatory care coordination and implement medical home pilots in partnership with payers

I wrote a big exclamation point next to page 362, where the authors wrote:
While much attention will be paid to organizational structure, it is best to think of the structure like a sports arena. It's the venue, not the game. Organizational structures do not ensure success, though an ineffective structure can make success much more difficult to achieve.

I encourage all healthcare organizations to buy this book, make reading each chapter part of a monthly ACO journal club, and above all, to use the process to build a safe environment for reflection and learning that will improve care for our communities.

Index.

It was very much need. And I enjoy using it a lot. So much helpful to use. While doing my studies
