This volume is the authoritative presentation of Dialectical Behavior Therapy (DBT), Linehan's comprehensive, integrated approach to treating individuals with borderline personality disorder. DBT--which has since been adapted for other difficult-to-treat disorders involving emotion dysregulation--combines cognitive and behavioral strategies with elements of psychodynamic, strategic, and other modalities. Delineated are specific strategies for contingency management, exposure, cognitive modification, and skills training.

See also Linehan's DBT Skills Training Manual, Second Edition, which provides essential materials for implementing DBT skills training, plus DBT Skills Training Handouts and Worksheets, Second Edition. Also available: Linehan's instructive skills training videos for clients--Crisis Survival Skills: Part One, Crisis Survival Skills: Part Two, From Suffering to Freedom, This One Moment, and Opposite Action. As soon as our county mental health clinic applied Dialectical Behavioral Therapy (DBT), our re-hospitalizations, crisis contacts, suicidal behavior and recidivism rates for our DBT patients went close to ZERO. This is the book I recommend as the start for anyone wanting to be effective in doing psychotherapy - including Psychiatrists, Psychologists, Social Workers, Marriage Family Therapists and Nurses. It empowers the therapist by giving him/her the skills to help severely mentally ill and difficult patients - not just ones with borderline personality disorder. For many, if not most, mental health programs, people with borderline personality disorder are traditionally frustrating, maddening, and looked on with disgust by therapists and medical staff. They are often rejected by staff and treated with anger because of the lack of adequate treatments for the disorder. Yet this is one of the most common mental illnesses. And persons with the disorder repeatedly are hospitalized for suicidal behavior - at large cost to the counties involved. Or worse, they are rejected for hospitalization and allowed to continue to be self-destructive. With DBT this is no longer the case. Therapists who understand DBT are confident and assured when helping seriously ill, constantly hospitalized patients. Therapist who don't often are frustrated, and rejecting of them. No other textbook in therapy is as detailed and well-delineated as this book. It is applicable to inpatient, outpatient, and emergency room settings. DBT works effectively in emergencies, with actively suicidal patients, to reduce the acuity of the situation. It is effective even in short (< 7 day) hospital stays. It takes about ONE YEAR to moderately understand Dialectical Behavioral Therapy.

Why is the gap so great between our hopes, our intentions, even our decisions-and what we are actually able to bring about? Even when we are able to make important changes-in our own lives or the groups we lead at work-why are the changes are so frequently short-lived and we are soon back to business as usual? What can we do to transform this troubling reality?

In this intensely practical book, Harvard psychologists Robert Kegan and Lisa Laskow Lahey take us on a carefully guided journey designed to help us answer these very questions. And not just generally, or in the abstract. They help each of us arrive at our own particular answers that can solve the puzzling gap between what we intend and what we are able to accomplish. How the Way We Talk Can Change the Way We Work provides you with the tools to create a powerful new build-it-yourself mental technology.

The Authors 235

Index 237

Kegan and Lahey explain that their book "is about the possibility of extraordinary change in individuals and organizations. It locates an unexpected source of boundless energy to bring these changes into being" and then assert that "if we want deeper understanding of the prospect of change, we must pay closer attention to our own powerful inclinations not [italics] to change. This attention may help us discover within ourselves the force and beauty of a hidden immune system, the dynamic process by which we tend to prevent change, by which we manufacture continuously the antigens of change." I am convinced that most human limits are self-imposed...that in Pogo's words, "We have met the enemy and he is us." The authors do indeed focus on what they call "an unexpected source of boundless energy" which significant change requires.
Throughout the book, they examine what they call "Seven Languages for Transformation" and suggest how to gain fluency in each. Four are Internal Languages: Commitment, Personal Responsibility, Competing Commitments ("Diagnosing the Immunity to Change"), and Assumptions We Hold ("Disturbing the Immunity to Change"). Fluency in these four enables us to build "The New Machine." There are also three Social Languages: Ongoing Regard, Public Agreement, and Deconstructive Criticism. Fluency in these three enables us to maintain and upgrade "The New Machine."
It is important to keep in mind that we communicate with others as well as with ourselves in three primary ways: body language, tone of voice, and content (ie what we verbalize). Decades of scientific research reveals that, in face-to-face contact, body language has the greatest impact, followed (at a significant distance) by tone of voice and then content.
Did you ever hear of something called "secondary gain"?
"Secondary gain" is the "hidden", possibly unconscious, reason why a person acts in a way that may, to an external observer, appear to be self-defeating. For example, Joe Bloggs frequently, and apparently sincerely, expresses a desire to lose weight - but he never does.
Why?
Because Joe has an unspoken belief that he will be safe from mugging so long as he looks big enough to wrestle a bull.
This isn't exactly rocket science. The genius of this book is that Kegan and Lahey have taken the "secondary gain" principle and repackaged it (without the usual psycho-babble) in a way that, hopefully, will appeal to the business community at large.
To this end they have developed a means by which people can quickly and easily - if they are willing - uncover what the authors call the "competing commitment" that undermines a person's declared commitment in a given situation.
For example, manager Fred Katz has the declared commitment of empowering his subordinates. Yet he briefs his people on a strictly "need to know" basis (and of course only Fred knows what his people "need" to know).
Using Kegan and Lahey's approach, described in detail in this book, Fred might discover that he has a competing commitment to gain promotion by demonstrating his indispensability. This he can only achieve, as he sees it, by keeping his people dependent on him as the one person in the department who has access to the "big picture".
Will this self-knowledge guarantee that Fred changes his behaviour?
Not necessarily. But at least he has a better understanding of his situation and is in a position to look for ways of achieving *both* commitments (empowerment AND promotion) in a constructive and non-conflicting manner.

Strictly off limits to the public, Plum Island is home to virginal beaches, cliffs, forests, ponds -- and the deadliest germs that have ever roamed the planet. Lab 257 blows the lid off the stunning true nature and checkered history of Plum Island. It shows that the seemingly bucolic island in the shadow of New York City is a ticking biological time bomb that none of us can safely ignore.

Based on declassified government documents, in-depth interviews, and access to Plum Island itself, this is an eye-opening, suspenseful account of a federal government germ laboratory gone terribly wrong. For the first time, Lab 257 takes you deep inside this secret world and presents startling revelations on virus outbreaks, biological meltdowns, infected workers, the periodic flushing of contaminated raw sewage into area waters, and the insidious connections between Plum Island, Lyme disease, and the deadly West Nile virus. The book also probes what's in store for Plum Island's new owner, the Department of Homeland Security, in this age of bioterrorism.

Lab 257 is a call to action for those concerned with protecting present and future generations from preventable biological catastrophes.

Reading an official press release from Plum Island is fairly benign. What's not easy is to read, however, is the litany of diseases handled incompetently there: Ebola, Marburg hemorrhagic fever, foot-and-mouth disease, Rift Valley fever virus, anthrax, plague, Entebbe, Zagazig 501, typhoid, diphtheria, E. coli, AIDS, polio, Japanese encephalitis, swine flu, mad cow disease, virulent influenza, Coxsackie B-5 virus, louping ill, Venezuelan equine encephalitis, Newcastle disease, vesicular stomatitis virus, contagious ecthyma, Nairobi sheep disease, and anthrax. Not all of these viruses have been contained. And that doesn't even take into account the various other test tube concoctions that cause encephalatic brain seizures, massive hemorrhaging, blindness, and death. Scary stuff, right?

Indeed, it's easy to make connections between Plum Island and three major disease outbreaks (West Nile virus - 1999, Lyme disease - 1975, Dutch duck plague - 1967). When looking at a map that displays the number of Lyme disease cases in the US, the results are staggering. There is a heavier concentration of Lyme disease within a small radius of Plum Island than anywhere else in the U.S., which doesn't come as much of a surprise when considering they bred and infected deer ticks and Lone Star ticks by the thousands.

Multiple outbreaks and deficiencies have been noted, but management has admitted only six employees contractions of exotic animal diseases: 3 - Newcastle, 2 - vesicular stomatitis, 1 - foot-and-mouth disease. On the verge of an environmental catastrophe for decades, it's hard to believe their miniscule estimates.
Like THE RIVER, author Michael C. Carroll has a lot of evidence some of it circumstantial and some of it solid that suggests the spread of Lyme Disease and even West Nile Virus could be the work of the US Government's research on Plum Island. Nestled in Long Island Sound off the coast of Connecticut and not far from New York City this "biological Three Mile Island" was established to do research on bio warfare using techniques that were developed by the Nazi's and Japanese during World War II. Using Project Paperclip to offer refuge to German scientists (and Japense as well) after the war U.S. scientists benefited from the knowledge that was developed on POW's and concentration camp victims to develop new and better ways to kill and disable our enemies.

Lab 257 was the focal point of much of this research. The lab had consistent failures in isolating many of the viruses and bacteria that they studied. In fact the negative pressure environment where the animal bodies were to be destroyed was wired incorrectly blowing potential viruses and bacteria everywhere prior to their disposal. Using information from people who worked there (some anonymously), declassified files and other credible sources of information Carroll makes a compelling case for the carelessness of our government in research that may have helped spread Lyme disease infected ticks and other viruses into the environment.

While it certainly is important to understand how many of these viruses and bacteria work, it's even more important to protect the public from contamination. The sloppy methods used at the lab put many people that live in the surrounding area at risk. Since takne over by the Department of Homeland Security, Americans are in the dark as to the plans for this biological waste disposal site.

Review

"Every once in a very long while in our field, a clinical innovation is introduced that profoundly improves patient care. Marsha Linehan's development of a cognitive-behavioral approach to borderline personality disorder is such a rare innovation....Her techniques are clear, teachable, and learnable, and make good common sense to the therapist and the patient. Dr. Linehan's methods have greatly improved my treatment of borderline individuals and my teaching of others in how best to understand and treat these patients.'--Allen Frances, MD

"Marsha Linehan's tremendously in-depth and informative book and its companion skills-training manual offer a needed guide to the cognitive-behavioral treatment of borderline personality disorder....Her method has become so widely used by clinicians that the experts in psychosocial treatments consider it one of the best....Marsha Linehan's methods are highly eclectic because they are empirically based; they have been fashioned to fit what works best for borderline patients."--Lester Luborsky, PhD

"Linehan is one of our most creative and articulate clinicians....As a true scientist-professional, she has not only been innovating in the treatment of one of the most daunting of disorders--borderline personality--but has successfully subjected her ideas to controlled empirical scrutiny."--Gerald Davison, PhD, University of Southern California

"Dr. Linehan's approach is sensible, pragmatic, and effective. Most importantly, it offers the therapist a humane and sensitive perspective on persons with Borderline Personality Disorder. In short, it treats both the therapist and the client in a fashion that produces results."--Mark Ciocca, PhD, Central New Hampshire Community Mental Health Center

"Linehan is a brilliant, compassionate, and rigorous therapist for borderline patients. What's more, and what is most unusual for master therapists, she has proven its efficacy in published randomized, controlled trials. What is fortuitous for borderline patients and their therapists is that she has painstakingly crafted a wise book and user-friendly manual well grounded in principle, detailed in its strategies, and exportable to a wide range of treatment settings. Our hospital has applied Linehan's treatment in several settings over the past five years: acute inpatient, extended inpatient, day treatment, and outpatient. We have primarily worked with borderline clients, but have also had success with some schizophrenics. Patients, families, and staff have received this new approach with enthusiasm; it makes common sense to them and feels very practical. It's a treatment that integrates well with biological, rehabilitative, most psychodynamic, and family treatment modes, and parallels treatments for eating disorders, substance abuse, and PTSD from childhood abuse....Linehan has set a high standard of treatment for borderlines in the 1990s; the patients will be the ultimate beneficiaries."--Charles R. Swenson, MD, New York Hospital, Westchester Division

"We highly recommend this text and its companion training manual...."--Bulletin of the Menninger Clinic
(Bulletin of the Menninger Clinic 1993-05-16)

"...represents a gold standard" for treatment of this diagnostic category."--Behaviour Research and Therapy
(Behaviour Research and Therapy 1993-05-16)

"It is the most exacting and well thought out treatment plan I have ever seen written for any disorder....I have tried to find shortcomings with this book. I can find none. It is one of the best pieces of clinical work ever written. No matter what your clinical or research focus of interest, you will gain immensely from reading Cognitive-Behavioral Treatment of Borderline Personality Disorder."--Journal of Family Violence
(Journal of Family Violence 1993-05-16)

This is a good book from a Medical Assisting standpoint. But, whatever you do, do NOT install the MOSS CD that's included with the book if you have MS Office 2010 (specifically Access 2010) installed on your computer. This will cause a MAJOR conflict and you will no longer be able to get to your personal Access 2010 files. Delmar/Cengage is aware of the problem, but has no solution as of the writing of this review. My class required the MOSS software and now my personal databases are messed up. The book is good and the premium website is great! Just be aware of the conflict between the MOSS CD and an install of MS Office 2010! Also, some of the Job Skills in this book are VERY confusing and the forms are so small you can't write in them.

Wow. I can't even believe that I'm writing this about such a major publishing company as Elsevier, but I am. This book is simply AWFUL. There are mistakes everywhere. Print mistakes. Copy mistakes. Editing mistakes. Informational mistakes. Two sentences one after the other that contradict each other. Here's an example sentence that has me completely baffled/astonished: "Prokaryotic cells are those containing no defined nucleus and a series of organelles that carry out the functions of the cell as directed by the nucleus." Huh? In the same sentence, they say it has no defined nucleus, and that the nucleus (which it doesn't have?) carries out functions of the cells. Wrong. Here's another great one: "HESI Hint: Concentrations have a great deal to do with the rate of reaction. As is the case with males and females, increasing the numbers of reactants (persons) means that a greater number of collisions can take place." I don't know whether to laugh or cry.

Sometimes, in the sample questions at the end of a chapter, they'll tell you that, say, "B" is the answer -- and then CONTRADICT that answer in the explanation. So, say, question #6 from the "Biology" section: "In which organelle does transcription begin? A: Ribosome; B: Nucleus; C: mRNA; D. Cytoplasm" The answer? C. Except that mRNA is not an organelle. So the description says "The DNA of a cell is located in the nucleus. Therefore the beginning of the transcription takes place in the nucleus." Um...wasn't that answer B? So now I'm TOTALLY confused about where transcription takes place, so I'll have to Google it.

They also have problems with superscripts and subscripts. So 6.02 times 10 to the 23rd (where 23 should be a superscript) is printed in the book: 6.02x1023. Great.

In this concise and practical review, the authors recognize that among the range of cardiac investigations available to veterinarians, the standard electrocardiogram (ECG) is an indispensable, safe and inexpensive test in assessing dogs and cats with heart disease.

Following discussion of the principles of electrocardiography, the book systematically explores the evaluation of the ECG—including determination of heart rate, measurement of intervals, derivation of the mean electrical axis, and criteria for atrial and ventricular enlargement or hypertrophy. It also examines intraventricular conduction disturbances and both normal and abnormal cardiac rhythms. Flow charts are provided to help users diagnose arrhythmias with confidence and 46 real cases and ECG tracings reinforce the principles and encourage discussion.

Rapid Review of ECG Interpretation in Small Animal Practice is illustrated throughout and is of value to all veterinary practitioners, technicians, and students who wish to improve their skills in interpreting ECGs.

This book is a completely new, revised, and expanded edition of this work. It now includes many more important and useful statements of fact, Chinese characters for every statement as well as Pinyin Romanization for every statement. This new edition also includes Bob Flaws's commentary explaining the clinical meaning of each statement. At last, what every TCM student has been looking for, a linguistically accurate, succinct list of the key statements of fact in TCM which, as a style of Chinese medicine, is largely a word game. However, to be effective at playing the game, one needs to know the words. Hopefully, this book will help Western students gain both clarity and proficiency in the process and practice of doing TCM.

I understand what Bob Flaws was shooting for with this book. Chinese medicine is built upon short statements that have withstood the test of time, although they come from many sources throughout the long history of traditional Chinese medicine. However it is only a little booklet full of these statements.

If I had my druthers, I would take each one of these "statements of fact" such as "Liver stores the blood" and add a clinical application such as "because the Liver stores the blood, it is implicated in menstrual irregularities or any condition that is coincidental with the menstrual cycle." Something like that.
By Al L. Stone
Statements of fact are phrases that help students and practitioners of Chinese medicine remember principles of their art. Often written originally in verse, these statements help keep us from straying too far from the root of our medicine and help organize our thinking about health and disease. Such statements are one reason that some Chinese practitioners are able to think their way out of wierd situations. They can constantly go back to first principles, then extrapolate in a creative way how to apply those to situations outside of usual textbook descriptions. We practitioners in the West should also endeavor to keep these principles at hand.
This book is a means to do just that. This new edition is a little larger than the previous edition, and it provides the original Chinese statements along with the English translation. Really interested folks can then make stabs at looking up the meanings of the original and can not only see what the translators had to go through, but can also absorb bits of the multiplicity of meanings in what seems at first to be simple statements. I've been in practice for several years and I still like to carry this around with me to keep the central ideas fresh and cement my understanding of first principles. If you practice or study Chinese medicine, you should own this book.

This is my first book review on Amazon, and that is really a tribute to this book. Despite having gone to a reputable medical school, CV physiology was taught to me as a series of overly-detailed lectures that made it hard to construct the forest from the proverbial trees. Since then, now a resident in Internal Medicine, and having been inspired to go into the field of Critical Care, I knew that I couldn't let this fractured knowledge-base continue for much longer. I had come across the author's website during med school (cvphysiology.com) and recently revisited it. I then bought this book and read it cover-to-cover. Out of the many reasons to praise his work, I think my vote goes to his ability to explain concepts WITHOUT simplifying them. Although paradoxical-sounding, this separates his book out from the rest of the sea of possible purchases. It is easy to make concepts understandable but so simplified that they are literally not clinically useful (e.g., the Made Ridiculously Simple series). On the other hand, probably more common, there are books which lose sight of the fact that the reader is learning, not reviewing what is already known. CV Physiology Concepts finds the balance, and in the end, is clinically very applicable.

To Dr. Klabunde, thank you for your authorship of such a useful tool for med students, residents, and seasoned physicians, to better grasp the important aspects of CV physiology. Certainly, I will be spreading the word about your book to my colleagues.

Amazing CV physiology textbook I have ever read. This is so nicely summarized, reasonably up-to-date, and yet without excess.
Easy to read, yet accurately written without losing essence. No problem for foreign language speakers.
Best for students, residents, and fellows who cover CV medicine, such as CV anesthesiology, CV surgery, and cardiology.
Also good for reviewing recent concepts in CV physiology as teaching staffs.

Bob Levoy practically invented practice management, but God is not finished with him yet. His latest book may be the best book on dental practice management ever written. We liked it so much we've recommended that it be the required textbook for our practice management course at Temple University School of Dentistry.
By Patrick Wahl
This book covers a subjest matter which is not black and white and does reasonably well. I have been through professional consultants a few times and this on a very basic level summarises the common recommendations that are put forward. The same problem exists though, that every practice is different, and they are only suggestions rather than specific rules for success. I would say, worth a read, though don't expect to see your practice thrive unless you have been living under a rock for the last few decades.

I am reviewing the ninth edition (2006) of Dr. Baroody's book, Alkalize or Die. Dr. Baroody's book has helped me understand the relationship between acid-forming foods and alkaline-forming foods, yet I must say it required some level of discernment to sift the wheat from the chaff.

One of the positive features to Dr. Baroody's approach to healthful living is his recognition of each person's unique biochemistry. Throughout this book he continues to remind his readers that each person is an individual and therefore might experience variant results when compared to other people.

It is important to understand that Dr. Baroody's theories and principals are based as largely on his personal alternative health experiences (trial and error) as they are on any demonstrable medical science. It is also important to know that Dr. Baroody gives much credence to the metaphysical teachings of eastern spiritualism, which may be a detractor to some readers. In the book's introduction, Dr. Baroody writes, "My viewpoint is founded on ancient Hindu, Chinese, Tibetan, and Greco-European healing philosophies and on several great modern men in the field of health." One of these "great modern men" is Edgar Cayce, who is referenced as an authority many times throughout the book.

So long as the reader is a proponent of New Age and/or Eastern spiritualism, this book should be quite acceptable. For those of us who are more Western in their worldview, the book requires discernment to pick and choose what you care to take from it.

An example of how Dr. Baroody's metaphysical beliefs influence his dietary advice can be seen in his counsel for reducing the consumption of red meat: "The destructive emotion of slaughter is assimilated by the person who eats flesh." (pg. 63).

Can't speak more highly of the kindle version of William's gyne - the textbook itself is classic, and written in a straight-forward, clear, concise manner, giving you everything you need to know and enough of what you want to know without inundating you with mindless details. The book is well-organized with a handy surgical atlas at the back, and overall, it's a great textbook for either a resident or i suspect even a generalist. the fantastic part of the kindle, however, is that it transforms the heavy, bulky textbook into something portable, and immediately searchable. i've personally read so much more since buying the electronic version; it's easy to pick a chapter or even a sub-chapter heading to peruse while you have an extra 5 minutes. i love williams, and the kindle version only makes it that much more accessible.

Patient Assessment, Intervention and Documentation for the Veterinary Technician addresses the methodology used to apply substantive information to real world problems in the education of veterinary technicians. Nursing students in human medicine have long benefited from instruction in the Nursing Process, which is a method for using critical thinking skills to apply theoretical knowledge to actual cases. This text develops a similar methodology for veterinary technicians, called the "Technician Practice Model". This guide identifies and describes the various steps in the Technician Practice Model and provides both instruction and examples on documenting the results of that process. Particular emphasis is given to developing SOAP notes and Medication Administration/Order Records (MAOR).

Design Concepts in Nutritional Epidemiology
Download electronic versions of selected books Design Concepts in Nutritional Epidemiology (Oxford Medical Publications) [Kindle Edition] for everyone book mediafire, rapishare, and mirror link In examining the relationship between nutritional exposure and disease aetiology, the importance of a carefully considered experimental design cannot be overstated. A sound experimental design involves the formulation of a clear research hypothesis and the identification of appropriate measures of exposure and outcome. It is essential that these variables can be measured with a minimum of error, whilst taking into account the effects of chance and bias, and being aware of the risk
of confounding variables. The first edition of Design Concepts in Nutritional Epidemiology presented a throrough guide to research methods in nutritional epidemiology. Since publication of the 1st edition, we now have a much better understanding of the characteristics of nutritional exposure that
need to be measured in order to answer questions about diet-disease relationships. The 2nd edition has been extensively revised to include the most up-to-date methods of researching this relationship. Included are new chapters on qualitative and sociological measures, anthropometric measures, gene-nutrient interactions, and cross-sectional studies. Design Concepts in Nutritional Epidemiology will be an essential text for nutritionists and epidemiologists, helping them in their quest to
improve the quality of information upon which important public health decisions are made. Direct download links available for Design Concepts in Nutritional Epidemiology

this book has good information for the design of a nutritioal epidemiologic study but it is more basic than Willett's Nutritional Epidemiology and is not as comprehensive. Contains more information on the practice of collecting information and some on basic epidemiology. If you already know epidemiology fairly well then you may want to opt for Willett's book, but if you are newer to the field or want more choices, this is probably a good book for you.

The revised LCME standards for accreditation include definitive language for diversity activities that institutions must implement by July 2015. This places greater pressure on medical education programs to implement policies and practices, as well as programs and partnerships, that will achieve diversity and inclusion goals. Failure to address these issues can result in punitive measures, such as being place on academic probation and loss of accreditation. Failure to advance diversity and inclusion also negatively impacts the institution’s ability to eliminate health and health care disparities and to advance health equity.

This guide, produced by a multi-disciplinary team of experts, provides a roadmap that will not only help you to comply with important diversity standards, but also to pave the way for developing and sustaining a climate of inclusion at your institution.

Part 1 of the guide provides the nine essential tasks as a roadmap for the process of strategic planning, plus offers key questions to help you think through each task. Part 2 identifies specific strategies that could be included in a plan to meet diversity and inclusion goals, tips for working with leadership, and the skills and competencies needed to succeed as a diversity officer.

Discover:

  • Guidance on setting objectives, identifying tasks, and developing action steps
  • Tips to help you avoid missteps that could slow your progress
  • Why diversity and inclusion should be included in strategic planning, including the case for excellence, accreditation, business and legality
  • Steps you can take in advance to avoid your diversity initiatives from becoming “siloed”
  • Tips to establish clear communication and decision-making channels with the key stakeholders
  • How to achieve buy-in to the timeline for carrying out a diversity initiative
  • Guidance on how the diversity officer’s role should be positioned to have authority and joint accountability to orchestrate meaningful change
  • Why dedicated diversity offers is a necessary pre-requisite for implementing any strategy for diversity
  • The unique set of leadership skills that diversity officers must have or must cultivate to be able to build the trust and credibility needed to move the work forward.

This resource will help to make the process of preparing a diversity and inclusion strategic plan more manageable and productive.
